Medic: Pacific War does not let Mayers save every casualty. Pearl Harbor places two wounded soldiers on the hospital approach and permits one extraction; the Philippines sniper sequence makes the same kind of choice. A useful run is therefore not built around touching every marker. It is built around recognizing which soldier can be moved, which objective advances the battle, and when treatment in the open will only create a second casualty.
This guide covers that decision loop, the reliable checkpoints in the two Early Access campaign chapters, and a patch-safe way to spend progression points. It was checked on July 18, 2026 against Steam AppID 1281640 and the official 0.2.1 notes from July 14. The developers are still changing balance, dialogue, challenges, collectibles, perks, and medical systems, so the live objective, current Field Guide, and current tooltip take priority over launch-week wording.

A rescue is four decisions, not one treatment
Before committing to an interaction, answer four questions in order: Is the ground about to be hit? Is the casualty visible to a gunner? Do I have the required supply? Where is the handoff point? The campaign repeats those questions with red strike zones, sniper indicators, machine-gun lanes, rubble that forces dragging, and stations that require a final stretcher interaction.
- Clear the hazard first. A red area under Mayers or a casualty is a warning to move, not a request to finish the treatment faster. Drag the wounded soldier beyond the marked strike area before opening the procedure if the game allows it.
- Break the enemy sightline. In the Philippines, wait behind sandbags until the sniper indicator drains. The Logistics Company Commander should be pulled out of sniper view before treatment. Do not confuse a nearby objective with a safe objective.
- Stabilize before the long carry. Follow the current treatment prompts and use the in-game Field Guide when an injury is unfamiliar. The Field Guide arrived in patch 0.1.3 and is the better authority for a changed procedure than a pre-release list of wound names.
- Finish the delivery. A rescued soldier normally must reach an ambulance, aid station, medical post, or radio tent. Dropping him near the marker may not complete the task; look for the designated stretcher or handoff interaction.
Restock whenever the route puts a supply box beside cover or before a long marked objective. The important habit is not hoarding a particular item count; it is refusing to leave a known resupply point with an unfinished route ahead. Patch 0.1.3 also renamed the old on-screen Score label to Supply Points, so a launch walkthrough using “score” is describing an older UI.
Pearl Harbor makes the nearest casualty the wrong choice three times
The prologue begins as a follow-and-treat lesson, then deliberately breaks the assumption that every wounded soldier should be handled in arrival order. The first route to the hospital has three places where location matters more than proximity.
From the pier to the ambulance
- Follow Philips from the pier, crawl beneath the rubble in the destroyed hangar, and use the first wounded soldier to learn the treatment interface. This soldier only needs to be stabilized before the route continues.
- After the next explosion, use the nearby supply box and stabilize both soldiers. Past the following gate, check the supply boxes on the left before treating the wounded soldier who introduces the tourniquet.
- Carry that soldier toward the ambulance. Rubble blocks the clean carry line, so drag him through the low obstruction, pick him up again on the far side, and place him on the stretcher.
- Take the medical crate after the truck cutscene. Red ground marks show the path of an incoming strafing run; leave the mark before trying to preserve a straight running line.
- After Philips sends Mayers onward alone, find the casualty beside the burning barrel. The strike marker is already forming beneath him. Drag him out of the zone, stabilize him, and carry him to the medical station.
- Treat the next marked soldier and continue to the gate by the tent. Beyond it, the route presents two wounded soldiers but allows one rescue. Choose, carry that casualty through the next cutscene, and deliver him to the ambulance. Searching for a hidden way to carry both only wastes the decision window.
The sixth checkpoint matters because it disproves “save every soldier” routes. It is a campaign dilemma, not a failed sweep. Patch 0.1.3 specifically fixed a bug that could trigger Pearl Harbor’s dilemma too early; if the choice appears somewhere that does not match the hospital approach, confirm the client is current before restarting the chapter.

The airfield and port are timed traffic, not a casualty sweep
At the airfield, the Morale meter falls while Mayers is idle and rises when wounded soldiers are treated. There are two timed stages, and a Special Quest for an AA Gun Operator appears during the sequence. Keep moving between casualties, but do not let the meter lure Mayers into a red mortar or strafing zone. A safe rescue that lands is worth more than a treatment interrupted in the open.
When the timer closes the airfield section, follow the compass back to the Commander. The next route mixes logistics with medical work:
- Enter the Logistics Yard and move the medical boxes from the hangar to the medical tent.
- Wait for the soldier to open the small gate beside the Do Not Enter sign. Take an ammunition crate from Temporary Ammunition Storage to one AA gun, then repeat the trip for the second AA gun.
- Continue to the Third Medical Post. At the collapsed hangar, stabilizing the three trapped soldiers is an optional experience objective. Do it only while the route and supplies permit; it is not the radio mission.
- At the port, ignore the temptation to begin a general casualty sweep. Radio Operator Stinard is on the northwest side, behind the hangar near the wall. Stabilize him and carry him to the nearby Radio Tent to finish the mission.
The port is the clearest test of objective triage. Several wounded soldiers are visible, but Stinard is the casualty whose extraction restores the radio objective. Treating the crowd first is compassionate but mechanically wrong because the mission does not give Mayers enough time to convert every visible casualty into a completed rescue.
The Philippines changes the danger from red zones to sightlines
The Philippines chapter starts inside Malinta Tunnel with five wounded soldiers. Treat as many as the opening window permits, then follow Private Lewis to check the storeroom. A cave-in blocks the return path; follow Lewis through the small opening and loop outside toward the tunnel entrance. The supply truck at the first sniper section is both cover and a resupply point, so use both functions before searching for Colonel Howard.
Reach Howard without racing the sniper indicator
Move between sandbags only after the detection indicator has fallen. When it fills, the answer is cover, not a faster treatment. Howard then identifies two soldiers hit by the sniper, but Mayers can help only one. Stabilize the chosen casualty, carry him back toward Howard, stay with the group through the next exposed area, and place the soldier at the nearest aid station.
The return to Headquarters crosses machine-gun fire. Patch 0.2.1 reduced machine-gun reaction time across all maps, so do not memorize the length of an opening from a launch video. Watch the current burst, move to the next cover when the lane actually breaks, and enter Malinta Tunnel to trigger the Headquarters checkpoint.

Use the three battlefield stages as objective resets
After the Headquarters meeting, return to the battlefield. This section has three stages and keeps Morale active while snipers and machine gunners limit the safe lanes.
- After Stage 1: deliver the medical supply box to its marked destination before treating the next stage like an open-ended rescue loop.
- After Stage 2: repeat the supply delivery. Restock and identify the next protected route while the objective transition is clear.
- After Stage 3: locate the Logistics Company Commander. Drag him out of sniper sight before stabilizing him, then complete his extraction.
- Cleanup order: carry the three newly assigned soldiers to the nearest Aid Station, return through the tunnel, and report to the Lt. General.
The final ambush switches to an Evacuation objective. Return to the tunnel entrance and watch the timer at the top of the screen. The required checkpoint is three rescued wounded soldiers before time expires. Once the third delivery registers, report back to the Lt. General instead of risking the completed chapter on an unsupported attempt to clear every body still visible.
Spend points against the failure you can name
A permanent “best skills” ranking is unsafe in this Early Access build. The July 14 patch reduced the effect of Perceptive and increased its price, while the July 10 update added Lifeline challenges that award Perk Points and promised more skills, perks, maps, and a new currency system later. Exact prices and value judgments can become stale between patches.
Use the current tooltip and buy against a repeated failure:
- You lose time locating valid casualties: compare the current version of Perceptive or another awareness option with its live cost. Do not buy it because an older page calls it mandatory; 0.2.1 explicitly weakened the value proposition.
- You reach soldiers but fail during exposed interactions: favor the available option that shortens the vulnerable part of Mayers’ loop or improves the movement problem you are actually seeing. Re-test on the same stage instead of assuming the upgrade fixes every map.
- You run dry before the named objective: change the route first. Use the supply truck, hangar boxes, and medical posts already placed on the campaign path. A progression purchase should not compensate for walking past a full resupply point.
- You want Lifeline progression: complete the current challenges for Perk Points, then choose for that mode’s repeated waves. A Lifeline purchase is not automatically the best campaign purchase because the modes create different timing and objective pressures.
Do not rely on claims that the current tree has a fixed irreversible order or on named upgrades that do not appear in the live client. The official roadmap is explicitly changeable, and later medical, currency, and perk work remains in development. Screenshot the tooltip before spending if you want to compare results after a patch.
Know when the client, not the route, has failed
Patch 0.2.1 lists a known failure, especially in Lifeline, where Mayers may become unable to interact with wounds or treat any casualty. The developer’s temporary solution is to restart the game. If every wound interaction fails, do not burn supplies, rearrange the route, or keep repeating the treatment input; preserve the result you can, restart, and verify the same casualty again.
The same patch warns that collectible previews may not persist between campaign chapters. This guide does not publish a collectible count or location route because the supplied walkthrough evidence does not verify one for the current build. Use the 0.2 Collectibles menu as a client-side record, but expect the preview-state issue until an official fix lands.
