Path of Exile 2 Act 3 Walkthrough: Waterways, Utzaal, and Doryani

Act 3 has three points where progress can feel broken even when the game is working: the Matlan Waterways are submerged until the Machinarium chain is complete, the route to ancient Utzaal opens only after the Temple of Kopec sequence, and the Sacrificial Heart gives no reward until it is carried into Aggorat. Solve those dependencies in order and the act becomes a readable route rather than one enormous jungle search.

This walkthrough follows the post-0.5 campaign sequence and the August 2026 0.5.4f client boundary. Patch 0.5 altered the Act 3 area order and changed the Waterways interactions to pressure plates, with the water already drained for the final portion. Older guides that describe the previous lever loop should be treated as historical.

Mark three Jungle Ruins jobs before leaving the hub

Cross the Sandswept Marsh, establish the Ziggurat Encampment, then enter the Jungle Ruins. Find the waypoint near the submerged Matlan Waterways and speak to Alva. You cannot enter the Waterways yet. The useful Act 3 opening is to mark its entrance, complete the two optional permanent rewards nearby, and then take the Infested Barrens exit.

Mighty Silverfist: two weapon-set points

Mighty Silverfist awards 2 Weapon Set Passive Skill Points. His swings, slam, and rolling attack are all physical commitments; the mistake is dodging on first movement and having no answer when the hit lands. Keep enough distance to read the shoulders, roll after the attack commits, and punish the recovery. Low armour makes the roll especially unforgiving, so a waypoint return after a gear upgrade is reasonable.

Venom Crypts: an irreversible defensive choice

Enter the Venom Crypts near the Jungle Ruins waypoint, take its venom sample, and deliver that quest item to Servi. The quest offers one permanent modifier:

  • raise Stun Threshold by 25%;
  • raise Elemental Ailment Threshold by 30%; or
  • improve Mana Regeneration Rate by 25%.

This selection cannot be changed later. Do not click through it as if it were ordinary quest loot. Choose Mana Regeneration only when sustained skill cost is a real bottleneck; otherwise use the character’s actual deaths to distinguish stun interruption from elemental ailment pressure. Confirm the selected buff in the character interface before continuing.

Chimeral Wetlands: read the head that attacks

Move through the Infested Barrens to the Chimeral Wetlands. Xyclucian guards Jiquani’s Machinarium and signals elemental attacks by head: the center head produces a fire cone, one side releases lightning projectiles, and the other creates freezing breath and chilled ground. When the Chimera flies to a pillar at health breakpoints, break the supporting platform to create a stun instead of merely circling under incoming volleys.

The Wetlands also connect to the Temple of Chaos, an alternate Ascendancy trial. Opening the route does not mean the character must finish it immediately. If modifiers or encounter pressure repeatedly exhaust the attempt, take the waypoint, gain levels and gear through the campaign, and return with a defined upgrade.

Machinarium core accounting

Jiquani’s Machinarium is a resource-routing puzzle. Pick up each Small Soul Core before walking away from its mechanism. One opens the initial route and another opens Jiquani’s Sanctum; additional cores expose the optional Blackjaw branch and a loot room.

Do not skip Blackjaw, the Remnant merely because the Sanctum door is available. Defeating Blackjaw gives the Flame Core. Consume it for +10% Fire Resistance. His large slams and fire spells reward moving behind the committed attack; rolling repeatedly without reading the next cast can land the character in burning ground.

Inside Jiquani’s Sanctum, locate both Medium Soul Cores and place them in their separate generators. Speak to Alva when prompted, activate the encounter, and defeat Zicoatl. For the expanding triangular slam, the geometry matters: stay near a triangle vertex rather than in the broad face of the expansion. Take the Large Soul Core after the fight.

Return to the Jungle Ruins and use that Large Soul Core at the Stone Altar. The visible change to the Matlan route is the completion signal. If the Waterways remain inaccessible, check whether a Medium Core generator, Alva conversation, Zicoatl loot, or Stone Altar interaction was missed.

Matlan Waterways: step, observe, then cross

The current Waterways use pressure plates to drain sections. Activate a plate, watch which channel changes, and cross the newly available path before hunting another control. The final stretch is already drained under the 0.5 campaign changes; do not search indefinitely for an obsolete lever described by an older walkthrough.

Near the end of the route, take the optional Azak Bog branch before advancing to the Drowned City. It contains the act’s Spirit reward and is worth doing on every character that intends to reserve another persistent effect.

Ignagduk: prepare the arena before the pull

In the Azak Bog, activate all effigies around the Flameskin Ritual. The temporary Ignagduk’s Harvest protection substantially improves fire resistance for the boss. Gear or a Ruby Charm is still useful; the ritual is preparation, not immunity.

When Ignagduk rides the broom, guide the charge into a summoned bone wall to knock her down. In the fire-heavy phase, do not stand between paired flame walls without an exit. For the Infernal Corridor, decide quickly between moving behind the boss and retreating far from the V-shaped blast. After the kill, use the Gemrot Skull; maximum Spirit rises by 30. Collect the accompanying Uncut Spirit Gem as well. Returning the Ghastly Spear to Servi supplies a charm choice.

Drowned City side work has two different payoffs

The Drowned City introduces a moving teal drowning field that reduces vision and becomes lethal if the character remains inside. Cross it; do not fight stationary in it. From this zone, the main path reaches the Apex of Filth, while the Molten Vault is an optional but important utility unlock.

The Molten Vault is a timed equipment check. If Mektul repeatedly resets to the gold, improve damage before spending more attempts.

Molten Vault: follow the gold to the Reforging Bench unlock

Pull the control beside the molten-gold vat and follow the moving flow. Side rooms provide routes around blocked segments; two optional Frost Cores open additional gold chests. At the Sluice Gate, descend to Mektul, the Forgemaster.

Mektul is timed because molten gold advances along the aqueduct. Staying too far away prompts a leap or lets the boss return to the gold and release waves, wasting the available window. Dodge the charged line slam and explosive rune, then deal damage close enough to prevent repeated retreats. If the gold reaches the arena before the boss is near death, the actionable fix is more damage, a better-supported main skill, or a few more levels—not a different maze route. Return the Hammer of Kamasa to Oswald to unlock the Reforging Bench.

Apex of Filth: three mushrooms are a flask errand

Defeat the Queen of Filth and take the Temple Door Idol for Alva. The Red, Green, and Blue Mushrooms found in the area belong in the Cauldron Keeper’s cauldron and produce flask rewards; they do not block the temple. Keep moving out of the Queen’s cursed ground, particularly when her mobility makes it tempting to chase through a debuffed circle.

Temple of Kopec: shade is part of the path

The indoor sun applies a stacking burn while the character stands in its light. Travel from shadow to shadow and search the corners for stairs upward. Treat the shade boundary as terrain, not background art. Ketzuli’s fireballs and tracking effects are easier to read when the character is not also carrying avoidable sun stacks.

After Ketzuli, summon Alva and complete her interaction. The raised platform returns the route to the Ziggurat and creates the time gateway. Step through to Utzaal in the past; no ordinary jungle exit substitutes for this transition.

Viper Napuatzi: the arena gets smaller, so decisions must get shorter

Utzaal contains dangerous cold projectiles, large physical enemies, Viper Napuatzi, and the Sacrificial Heart. A Sapphire Charm or cold resistance helps against Eye of Winter casters on the approach. Farm gear here only if it produces a concrete improvement; Viper is a severe check, but unfocused clearing does not solve it.

Against Viper, learn the spear language:

  • Move out of horizontal poison puddles instead of attacking across them.
  • When the lightning-infused spear is thrown, expect projectiles to return after it reaches the shield wall.
  • The charged serpent line is unblockable; leave the line rather than relying on defence.
  • Ground runes precede the aerial storm. Keep moving through clear spaces, then prepare for larger chaos impacts.
  • When the shield guard attacks and closes in, stop using the old outer safe lane. The shrinking arena turns long attack animations into a liability.

Kill the remaining shield guards after Viper falls. Before leaving Utzaal, find and take the Sacrificial Heart. It does not grant its points on pickup.

Aggorat: finish the heart ritual before the Black Chambers

Carry the Heart into Aggorat. Find the blood-sacrifice altar and Sacrificial Dagger, place the Heart, then use the dagger on it. The ritual adds 2 Weapon Set Passive Skill Points. Reaching the Black Chambers with the Heart still in inventory means the reward is incomplete; turn back while the waypoint chain is fresh.

Doryani: move with the machine, then answer the caster

Doryani begins with elemental hand attacks, lines of exploding soul gems, targeted lightning, and a ground-slam nova. Watch which hand or ground marker starts the cast, leave its line, and use the recovery for damage.

At the mechanical transition, a central altar rotates pulsing lasers and charges spikes around the arena. Walk with a safe gap rather than rolling across every beam. Touching charged spikes triggers lightning arcs, so do not hug the wall blindly. Solar Orbs remain as delayed hazards and can be activated by later casts; move the fight away from accumulated orbs before taking a damage window.

If the second form overwhelms the character, separate the diagnosis:

  • Deaths while crossing lasers indicate pathing or premature dodge rolls.
  • Lightning spikes demand more awareness and possibly Lightning Resistance.
  • Solar Orbs covering the arena mean Doryani has been fought in one location for too long.
  • A fight that lasts through too many hazard cycles needs a weapon, gem, or passive-tree upgrade.

Act 3 handoff

Before traveling onward, verify Mighty Silverfist’s 2 points, the chosen Venom Crypts modifier, Blackjaw’s +10% Fire Resistance, Ignagduk’s +30 Spirit, the Reforging Bench, and the Sacrificial Heart’s 2 points. The Act 4 and Interludes route continues from Kingsmarch to endgame. If one reward is uncertain, use the current campaign bonus checklist instead of replaying the whole act.
