Path of Exile 2 Act 4 and Interludes Walkthrough: Island Order to Endgame

Act 4 gives the player a ship and several islands, so there is no single sacred island order. There is, however, a clean decision rule: secure permanent rewards and quest items when their island is active, return Torn Map Pieces as one set, and delay a damage-check boss only when the waypoint makes the return cheap. After Tavakai, the three Interludes form a dependent campaign chain and are best routed together.

This guide is bounded to Early Access patch 0.5.4f in August 2026. The Interludes are explicitly temporary campaign bridges while later acts are unfinished. Reward locations changed during Early Access: most importantly, the Blind Beast currently gives a Blank Greater Rune, while the Act 4 weapon-set points associated with that part of the route come from Omniphobia in Dark Mists.

Build an island board before choosing an order

From Kingsmarch, take the ship to Ngakanu and obtain the charter. Then use the table below as a job board. The order is a recommendation for reducing hand-in friction, not a requirement.

Stop Protected outcome Leave only after
Whakapanu Shark Fin and a permanent Lesson choice Great White One is dead and the fin is held or handed in
Shrike Island Search progression and a Torn Map Piece Matiki/Eye objective and island piece are checked
Abandoned Prison Switchable Goddess flask boon Prisoner is defeated and the selected flask effect is visible
Isle of Kin Blank Greater Rune Blind Beast reward is looted; do not expect passive points here
Kedge Bay Dark Mists reward and Freya rescue Omniphobia is defeated and 2 weapon-set points are received
Plunder’s Point Optional bounty Four Torn Map Pieces have been assembled first
Eye of Hinekora Mana, tattoos, and Yama points Silent Hall and desired tests are checked before sailing

A character that is struggling should front-load Whakapanu, the Prison, and Kedge Bay because they offer durable power or utility. A strong character can clear the four Torn Map Piece islands in any convenient order and resolve the optional bounty in one visit.

Whakapanu: decide the Shark Fin reward with the tooltip open

Find the Singing Caverns and kill the Great White One at the Sharkpit for the Shark Fin. Return it to Kaimana at Ngakanu, either immediately or when the act brings you back. Under the current campaign, Tribal Medicine offers a permanent Lesson choice rather than the old gem-only outcome described by early Third Edict guides.

  • Kaom’s Lesson is the broad defensive option, increasing Armour, Evasion, and Energy Shield.
  • Rakiata’s Lesson combines an Armour contribution against elemental damage, Deflection based on Evasion, and a faster start of Energy Shield recharge.

The choice is not reversible. Read the live item tooltip before consuming a Lesson because numerical balance can move between Early Access patches. Kaom’s is easier to use across mixed defences; Rakiata’s is a deliberate choice for a character whose layers actually exploit those conversions and timings.

Shrike, Prison, and Isle of Kin: three short jobs with different rewards

Shrike Island

Follow the nest-covered route, deal with the Shrike, and progress the search involving Matiki and the Eye. Recover the island’s Torn Map Piece. The boss’s aerial or obscured transitions are a cue to move and reacquire, not to continue attacking a target that cannot be safely hit.

Abandoned Prison

Clear toward the Prisoner and watch for his healing attempt. Use the arena’s ballista interaction to interrupt it; allowing repeated heals turns a manageable fight into an attrition loss. The Goddess reward changes how Life and Mana flasks behave. It is switchable by returning, so choose the effect that addresses the current flask failure rather than worrying about a permanent lock-in.

Isle of Kin

Kill the Blind Beast and take the Blank Greater Rune, which can be exchanged through the Kingsmarch vendors for an appropriate Greater Rune. Do not count this as a weapon-set point reward. Older Act 4 checklists assigned 2 points here; current evidence places those points in the Dark Mists quest at Kedge Bay.

Kedge Bay: free Freya, then finish Dark Mists

Advance the Search through Kedge, use the Verisium interactions, and free Freya Hartlin. The apparent rescue is not the end of the branch. Continue until Freya is revealed as Omniphobia, Fear Manifest, survive the Delirium pressure, and defeat the boss. Complete the return dialogue in Kingsmarch; it adds 2 Weapon Set Passive Skill Points.

If the boss dies but the points do not appear, check the return conversation before resetting an instance. The reliable completion test is the consumed reward and point count, not the disappearance of fog.

Island bosses vary widely. Keep the route anchored to quest items and permanent rewards instead of treating every encounter as the same kind of clear.

Assemble the optional pirate route only after four pieces

Torn Map Pieces are found across the Isle of Kin, Kedge Bay, Whakapanu, and Shrike Island. Collect all four before treating Forgotten Bounty as an active destination. Combining an incomplete set into repeated inventory checks wastes time; the quest panel and item count tell you whether Plunder’s Point is ready.

The bounty is optional and primarily a loot route. It should not displace the permanent Kedge, Whakapanu, or Eye of Hinekora work when a character is under-powered.

Eye of Hinekora: one secret, three choices, one point boss

In the Eye of Hinekora route, complete the tests of mettle and search near the late checkpoint for the Silent Hall. Pay respects inside to receive +5% maximum Mana. A cleared main corridor does not prove the secret room was visited; check the act-map icon and the character’s Mana total.

The Halls of the Dead contain three optional tests that yield Blank Tattoos. A tattoo converts into an attribute or resistance reward associated with its test, and consuming the final choice is permanent. Some community tables disagree on a test label, so use the reward window itself as authority: pause, read both offered outcomes, and record the choice before applying it.

Defeat Yama, the White and use the Silver Coin to enter the Trial of the Ancestors. At its summit, the follow-up dialogue with Navali adds 2 Weapon Set Passive Skill Points. Killing Yama without that conversation is not the final readback.

Arastas to Tavakai: follow the main dependency, not island completion percentage

On Arastas, defeat Torvian. He draws power from nearby casters; lure him behind the large rock formation to break their empowering line, then fight him without the stacked support. Continue the Benedictus route and follow Tavakai and Makoru to the weapon. Return through the Hooded One conversations as directed, then sail to Ngakanu for the act finale.

After Tavakai, speak through the Kingsmarch handoff until the Hooded One offers the Interludes. This is the real Act 4 completion signal. A remaining optional pirate bounty or switchable boon can be revisited, but the campaign path now continues through three temporary stories.

Interlude I: Serle’s six megaliths lead to Wolvenhold

From the Refuge, cross the Scorched Farmlands and resolve the paired boss encounter. In the Stones of Serle, activate all six megaliths around the area’s outer reaches. If the Blackwood route will not advance, count the activated stones rather than reclearing the center.

Continue through the darkness sequence and Blackwood to Wolvenhold. Defeat Oswin, the Dread Warden and use his Ledger. The character’s weapon-set allowance should rise by 2 points. Finish the Holten Estate duo and complete the Hooded One handoff to the next Interlude.

Interlude II: take the northern Life reward before splitting the route

At Khari Crossing, first go north to the Skullmaw Stairway and the hidden Molten Shrine. Use the Molten One’s Gift; it raises maximum Life by 5%. This detour is short enough that leaving it for cleanup usually costs more.

Defeat Akthi and Anundr, then report back to Risu. That conversation adds 2 Weapon Set Passive Skill Points; the boss kill alone does not. Next, cross the Pools route toward the Sanctuary and Elzarah, then proceed through Galai and Qimah.

At Orbala’s Pillar in Qimah, choose one persistent character buff. Its menu includes movement speed, global defences, elemental resistance, all attributes, cooldown recovery, a larger Presence area, and an experience option with inverted drawbacks. This selection is changeable by returning to the pillar. Inspect all tradeoffs before picking the experience modifier; faster leveling is not free when it reverses the other boons.

Finish the Qimah Reservoir and Azmadi, use the Grand Barya interactions, and return through Sekhema Asala and the Hooded One to unlock the final Interlude.

Interlude III: three permanent stops before the vault

Kriar Village: Lythara’s Spirit reward

Progress from the Ashen Forest into Kriar Village. Lythara becomes more dangerous as Azmeri wisps empower the encounter, so this is a damage check rather than a fight to prolong. Improve the main skill and weapon before entering if ordinary enemies already take too long. Use the Gemcrust Skull after the kill. It increases maximum Spirit by 40; collect the Uncut Spirit Gem too.

Glacial Tarn: return the Yeti item to Hilda

Defeat the Abominable Yeti and take the Icy Tusks. They belong to Hilda; their hand-in adds 2 Weapon Set Passive Skill Points. The Yeti’s heavy attacks resemble a timing exam: roll the impact, not the start of the animation.

Final Hooded One reward

Continue through Rakkar, the Peaks, the Etched Ravine, and Cuachic Vault. Elder Madox is optional but can sell useful unique items; visit him only if his inventory solves a defined build need. Once the vault chain is complete, go back to Kingsmarch. The final Hooded One conversation adds the remaining 2 Weapon Set Passive Skill Points; then travel to Oriath for the endgame handoff.

End-of-campaign audit

  • Tribal Medicine Lesson consumed after reading the current tooltip.
  • Goddess flask boon selected; Blind Beast rune collected.
  • Omniphobia hand-in complete: 2 weapon-set points.
  • Silent Hall complete: +5% maximum Mana; desired tattoos applied; Yama/Navali complete: 2 points.
  • Oswin ledger consumed: 2 points.
  • Molten One’s Gift consumed: +5% maximum Life; Akthi/Anundr hand-in complete: 2 points; Orbala boon selected.
  • Gemcrust Skull consumed: +40 Spirit; Yeti/Hilda complete: 2 points; final Hooded One reward complete: 2 points.

For reward locations without the story path, use the Acts 1–4 and Interludes permanent bonuses checklist. That page also separates irreversible choices from boons that can be changed later.

Evidence and Early Access boundary

The developer’s Steam product record supplied campaign status and the original images. Area and reward changes were compared with the developer’s 0.5.0 patch record plus current verified route references. The live quest panel and reward tooltip take precedence when a future Early Access patch moves a point reward or changes a Lesson value.
